How the Application Process Works

Download & Complete the Medical Form

Print the medical form, take it to your doctor, and have it completed and signed. This confirms you are medically fit to enroll and participate in physical work.

Agree to Terms & Conditions

Read and agree to our indemnity form and POPIA consent before submitting.

Submit & Await Confirmation

Submit your application. We will review it and contact you within 3-5 business days to discuss next steps and availability.

Sand River Recovery Centre NPC (NPO 325-355, NPC 2022/599569/08) is a faith-based residential recovery program and is NOT a medical facility. We do not provide medical treatment, emergency medical care, or clinical psychiatric services.

By submitting this application, I acknowledge and agree that:

Sand River Recovery Centre is NOT a medical facility and does not provide medical treatment.
I will not hold Sand River Recovery Centre, its staff, volunteers, or directors liable for any harm, damage, injury, loss, or death that may occur during my participation in the program.
I understand that participation in physical work and activities is part of the program and I accept the associated risks.
I confirm that I have obtained medical clearance from a registered medical doctor confirming I am fit to participate in the program and physical activities.
I understand that Sand River Recovery Centre reserves the right to discharge any resident who does not comply with program rules and guidelines.
I agree to abide by all program rules, guidelines, and the decisions of program staff.

POPIA Consent — Protection of Personal Information

In accordance with the Protection of Personal Information Act (POPIA), Act 4 of 2013, Sand River Recovery Centre NPC collects, stores, and processes your personal information for the following purposes:

Processing and evaluating your application for the residential program
Administering your participation in the program
Communicating with you and your emergency contact regarding your program
Maintaining records required by law and our NPO obligations
Facilitating sponsorship arrangements where applicable

Your information will not be shared with third parties without your consent, except where required by law. You have the right to access, correct, or request deletion of your personal information at any time by contacting us at [email protected].
